The documents Florida will ask you for

Primary ID, proof of your Social Security number, and two proofs of Florida address. What counts, what gets rejected, and the paperwork people forget.

Florida wants three things from you at the counter: one primary identity document, one document proving your Social Security number, and two separate documents showing your Florida residential address. Two address documents, not one. That single detail sends more people home empty-handed than anything else in the process.

Everything below comes from FLHSMV's own published requirements. Bring originals. Photocopies are not accepted, and neither is a photo of a document on your phone.

Primary identification

You need one original document from this category. For a U.S. citizen, FLHSMV accepts:

  • A U.S. birth certificate, including some U.S. territories and the District of Columbia — but only one issued by a county health department or the state Bureau of Vital Statistics
  • A valid U.S. passport or passport card
  • A Consular Report of Birth Abroad
  • A Certificate of Naturalization, Form N-550 or N-570
  • A Certificate of Citizenship, Form N-560 or N-561

The hospital souvenir birth certificate with the tiny footprints is not a legal document. People bring it. It gets rejected. If you are not sure which one you have, order a certified copy from the state you were born in now, not the week you plan to test — some states take three weeks.

Proof of Social Security number

One original document showing your complete name and your full SSN. FLHSMV accepts the Social Security card itself, a W-2 that is not handwritten, a pay stub showing the number, an SSA-1099, or any other 1099 that is not handwritten.

Note what is not on that list: a photo of your card, a screenshot of a benefits portal, or a letter from an employer. If your card is lost, request a replacement from the Social Security Administration before anything else — it is free, and it takes time.

If your name on the SSN document does not match your name on the primary ID, you need the connecting paperwork: an original or certified marriage certificate, divorce decree, or court order. Photocopies are not accepted here either, and a mismatch between three documents is the second most common reason people get turned away.

Two proofs of Florida residential address

This is the one. FLHSMV's requirement is that "customers must submit two different documents with their Florida residential address." Different documents, both in your name, both showing the address you actually live at.

Commonly accepted examples include a lease or rental agreement, a deed or mortgage statement, a utility bill, a vehicle registration, a voter registration card, an insurance policy or bill, a bank statement, or a pay stub showing your address. Florida county tax collectors publish their own accepted lists, and they vary slightly in what they will take as a second document, so it is worth checking your county's page as well as FLHSMV's what-to-bring list.

If you live with family and nothing is in your name, that is a real problem and it has a real fix. Florida has a certification-of-address process where the person you live with signs a sworn statement and provides their own two documents. Ask the tax collector office about it before you go, and bring the other person if you can. Do not show up hoping to talk your way through it.

What the CDL adds on top

Everything above is the baseline for any Florida license. For a commercial license you also need:

Your existing valid Florida driver license. Florida issues a commercial learner permit to people who already hold a regular Class E license, so if you are new to the state, get that done first as its own trip.

Your medical certification on file. Since June 23, 2025, certified medical examiners transmit DOT physical results electronically to FMCSA's National Registry, which forwards them to state licensing agencies. Florida offices no longer take a paper card across the counter as the way to register your medical status. FLHSMV's guidance says that once the certificate is submitted electronically, you are not required to carry a med card. What you should do is verify it actually arrived a few days after your physical — if it did not, here is how that gets fixed, and it is far easier to fix before you are standing in line.

Your self-certification. You pick one of four categories describing the driving you expect to do: non-excepted interstate, excepted interstate, non-excepted intrastate, or excepted intrastate. Most new Class A drivers choose non-excepted interstate. That category requires a current medical certificate on file, and choosing it wrongly is how a license quietly gets downgraded a year later.

Your entry-level driver training completion. You do not carry this one — your school reports it electronically to FMCSA's Training Provider Registry, and Florida checks the registry before letting you take the skills test. Build the folder before you build the schedule

Put all of it in one envelope a week ahead: primary ID, SSN document, two address documents, name-change paperwork if your names differ, and your current license. Then check the folder against the list one more time the night before, out loud.

This sounds excessive. It is not. A missing second address document costs you a day off work and pushes your permit date back, and the permit date is what starts the federal 14-day clock before you can take the skills test — which is the constraint that sets your whole permit-to-license timeline.

Do this next

Open the envelope test right now: pull out your primary ID and your two address documents and look at them. If any one of the three is missing, order it today. Ordering a birth certificate or a replacement Social Security card is free or close to it, and it is the only part of this process where waiting costs you nothing but time you do not get back.
